About this template

This school nurse log is a nurse visit log, health office log and first aid log in one. It records every visit to the health office with the pupil, class, time, reason and what happened next. It is for school nurses, first aiders and office staff who need a clear record and a quick summary of the term without reading every line.

Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).

Head injuries counted on their own

A head injury and a nosebleed are both a visit, but usually only one of them has to be reported. Head injuries are counted separately, so the file doubles as a head injury log, because that is the number that goes in the accident book and the one a head teacher asks for.

Parent contact in four states

Parent told is not a tick box. It is yes, tried, no or not needed, and tried is the state that matters at four o'clock.

Actions and medication

Seven actions are chosen from a drop-down: back to class, rest and review, parent called, sent home, ambulance, first aid given and medication given. They are counted, so a term can be summarized without reading it. The medication log records what was given and who gave it, the two columns most log sheets lack and an audit asks about.

  • 300 visits with pupil, class and time
  • Twelve reasons as a drop-down
  • Head injuries counted on their own
  • Seven actions, counted
  • Parent told: yes, tried, no or not needed
  • Medication given, and by whom
  • Back to class, recorded as a time
  • A Tasks tab for everything the main tab does not cover, and a dashboard reading all of it

Five tabs and a filled example

Start Here, Setup, Clinic, Tasks and Dashboard. The example copy holds 46 visits in a term, with six head injuries, nine doses of medication given, two pupils sent home and two parents nobody could reach. The blank copy is ready for your own clinic, and the Start Here tab walks you through setup in a few minutes.

This is a record-keeping sheet, not medical guidance, and it does not replace your school's accident book or safeguarding policy.

Frequently asked questions

Does it work in Excel and Google Sheets?

Yes. The Excel workbooks run on Excel 2016 or later on Windows and Mac, and the instruction guide has one-click links to copy the Google Sheets versions with a free Google account.

Can I record medication given?

Yes. Medication given is one of the seven actions, and each visit records what was given and who gave it.

How does it handle head injuries?

Head injuries are counted on their own, separate from other visits, so the figure for the accident book is always ready.

Does it replace our accident book?

No. It is a record-keeping sheet, not medical guidance, and it does not replace your school's accident book or safeguarding policy.

How many visits does it hold?

300 visits, each with pupil, class, time, reason, action and parent contact.
