About this template
This blood sugar log sorts your readings by when they were taken, because a waking reading and an after-dinner reading are two different conversations. It is for people with type 1 diabetes, type 2 diabetes or prediabetes who want their numbers organized for the next review.
Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).
Four things per reading
Each reading needs only the date, which part of the day, whether it was before or after a meal, and the number. Everything else in this glucose log is arithmetic on that tab, which has room for 1,400 readings.
- Low, in range or high, worked out on every row against your own target and colored
- Your target typed once on Setup, in your own unit
- After-meal readings judged against a different ceiling, as they should be
- Nothing is converted: mmol/L or mg/dL, stored exactly as you were given it
Patterns by time of day and day of week
The Patterns tab splits your readings across six times of day and seven days of the week, each with its own average, its own count in range and its own share. That split is where the pattern shows. A single monthly average is where it hides.
What a dose change did
On the Medications tab, a dose change is a new row rather than an edit, for example metformin 500mg from January and 1000mg from April. Each row compares the thirty days before the change with the thirty days after, which is the question a review appointment asks. With fewer than ten days logged before a change, it says so instead of guessing. Refill dates are calculated from the interval, so it also works as a medication tracker.
Results, appointments and the report
- Results: every HbA1c with the lab's range beside it and the change since the last one
- Appointments: what the nurse said, kept, with the date to come back
- Monthly: readings, average and share in range, twelve rows
- Report: one printable page to take to the review
The ten connected tabs are Start Here, Setup, Readings, Patterns, Medications, Appointments, Results, Monthly, Report and Dashboard, so the file works as a glucose tracker and a diabetes planner in one. It is a logbook that does arithmetic on the numbers you type. It does not set your target range, does not know your history and cannot tell you whether a reading matters.
Works with
- Microsoft Excel 2016 or later, on Windows or Mac
- Google Sheets with a free Google account. The Google Sheets version is a real workbook, not a PDF with a link in it.
- No dynamic arrays, no macros and no add-ins, so it behaves the same on Excel 2016, 2019, 2021 and Microsoft 365 and in LibreOffice. No Microsoft 365 subscription is required.

Frequently asked questions
Does it work in Excel or Google Sheets?
Both. The Excel files run on Excel 2016 or later on Windows or Mac, and the instruction guide has one-click links to the Google Sheets versions for a free Google account.
Does it work in mmol/L and mg/dL?
Yes. You use whichever unit you were given and nothing is converted. Your target is typed once on Setup in that unit.
Can it show whether a dose change helped?
Each dose change is its own row, with your readings for the thirty days before compared with the thirty days after. With fewer than ten days logged before the change, it says so.
Can I track HbA1c results?
Yes. The Results tab records every HbA1c with the lab's range beside it and the change since the last one.
Does it tell me what my target range should be?
No. It is a logbook that does arithmetic on the numbers you type. Your target range comes from whoever looks after your care.
