About this template
This therapy journal is a private place to keep your own notes from each session and carry them into the next one. It is your journal, not a medical record. Write what you talked about and what you want to remember, and the dashboard keeps the things to bring up next time in front of you.
Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).
The dashboard
Choose any topic and the page follows. It fills itself from your sessions.
- Sessions in the last 12 months, for every topic or one
- Mood before and after, month by month, as you rated it out of 10
- The notes you wanted to keep, newest first
- How often you met each month, and which topics come up most
- What you noted to bring up next time
- The next session booked, and how you will meet
- What you are practicing between sessions, with times done, and what you paid each month
Every session
One row a session holds the date, in person or by video, the main topic, what you talked about, what you want to remember and anything to bring up next time. These session notes are in your own words.
What you are practicing
The Between Sessions tab lists every thing you agree to try between sessions, how often, how many times you have done it and what you noticed. It keeps therapy homework from getting lost between appointments.
Mood before and after, if you want it
Rate your mood out of ten before and after each session, only if you choose to, and it is charted month by month. The mood tracker part is optional; the therapy notes work without it.
What comes up
The Topics tab shows how often each topic comes up and what sessions cost you a month. It works as a quiet self care journal alongside the work you do in the room.

Light and dark editions
Every download includes a light edition and a dark edition. Every tab is restyled, not only the dashboard, and both run on the same formulas, so you simply pick the look you prefer. Each edition comes filled with a worked example and as a blank copy, in Excel and in Google Sheets.
Works with
- Microsoft Excel 2016, 2019, 2021 and Microsoft 365, on Windows and Mac, and LibreOffice
- Google Sheets with a free Google account. The Google Sheets version is a real workbook you take a copy of, not a PDF with a link in it.
- No dynamic arrays, no macros, no add-ins and no subscription
What you download
- Light edition: the Excel workbook filled with a worked example (a year of sessions, 41 of them, and 12 things practiced), and a blank one
- Dark edition: the same two files in the dark design
- The instruction guide, which has one-click links to the Google Sheets versions (light and dark, filled and blank)
- The terms of use as a PDF
Your files are ready to download as soon as payment clears. They are for personal and business use on your own devices, so please do not share or resell them. Questions before or after you buy? Use our contact page and we will help.
Frequently asked questions
Can I keep it in Google Sheets?
Yes. The download includes Excel workbooks for Excel 2016 or later on Windows or Mac, and the instruction guide has one-click links to copy the Google Sheets versions with a free Google account.
Is this a medical record?
No. It is a private journal for your own notes, and it gives no advice. It records what you write and adds up what you enter.
Do I have to rate my mood?
No. Mood before and after, out of ten, is only there if you want it, and the rest of the journal works without it.
Can I track what I practice between sessions?
Yes. The Between Sessions tab holds each thing you agree to try, how often, how many times you have done it and what you noticed.
What is the difference between the light and dark editions?
Only the look. Both have every tab restyled with the same formulas, and both come with every download.
