About this template
This doctor appointment tracker is for the person who keeps track of a household's doctor visits, whether that is a parent or a caregiver. Log every visit, past or booked, and the file shows what is coming up, which follow-ups still need booking and what you have paid.
Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).
The dashboard
Choose anyone in the house and the page follows. It fills itself from your visits.
- The next visit booked, who it is for and where, and the next eight appointments
- Every visit of the last year by month, person by person
- The months ahead: booked visits and follow-ups to book, per person
- Your providers, with visits, last and next, and any referral running out
- What you paid, month by month
- Questions still to ask, provider by provider
- Why you went: checkups, follow-ups, new problems and more
Every visit, past or booked
Each row holds the date and time, the provider, who it is for, why and what you paid, so it doubles as a copay tracker. After the visit, add what was said and the date you were asked to come back by.
Follow-ups you still need to book
A come-back date stays on the list until a later visit is booked, soonest first, so nothing slips. Referrals are marked before one runs out, which turns the file into a simple referral tracker as well.
Every provider and every question
The Providers tab keeps each doctor, dentist, eye doctor or therapist with the phone number, visits, the next booking and what you paid. The Questions tab holds questions as you think of them, with the next visit where you can ask, so this medical tracker covers family health from one place.
Six connected tabs
Start Here, Setup, Appointments, Providers, Questions and Dashboard.
Light and dark editions
Every download includes a light edition and a dark edition. Every tab is restyled, not only the dashboard, and both run on the same formulas, so you simply pick the look you prefer. Each edition comes filled with a worked example and as a blank copy, in Excel and in Google Sheets.
Works with
- Microsoft Excel 2016, 2019, 2021 and Microsoft 365, on Windows and Mac, and LibreOffice
- Google Sheets with a free Google account. The Google Sheets version is a real workbook you take a copy of, not a PDF with a link in it.
- No dynamic arrays, no macros, no add-ins and no subscription
What you download
- Light edition: the Excel workbook filled with a worked example (26 visits for a family of four with eight providers), and a blank one
- Dark edition: the same two files in the dark design
- The instruction guide, which has one-click links to the Google Sheets versions (light and dark, filled and blank)
- The terms of use as a PDF
Your files are ready to download as soon as payment clears. They are for personal and business use on your own devices, so please do not share or resell them. Questions before or after you buy? Use our contact page and we will help.

Yes. You get Excel workbooks for Excel 2016 or later on Windows or Mac, and the instruction guide has one-click links to copy the Google Sheets versions with a free Google account.
Can I track appointments for the whole family?
Yes. Every visit records who it is for, and you can pick anyone in the house to see only their visits on the dashboard.
How does it remind me to book a follow-up?
Enter the date you were asked to come back by. It stays on the follow-ups list until a later visit is booked.

What is the difference between the light and dark editions?
Only the look. Every tab is restyled, the formulas are the same, and both come with every download.
