About this template
This caregiver log is for families looking after a parent or relative at home, where several people share the care and nobody quite knows what happened yesterday. Whoever was there writes one row at the end of the visit, and everyone else can read it.
It replaces the shared notebook with a record that adds things up: how good days and hard days compare, how many falls there have been, and whether a change in medication made a difference.
Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).
The daily handover
- Room for 300 days, nine columns a day, written in under a minute
- How they were, appetite, mobility and orientation
- Pain and sleep on the same row
- Falls as their own column, counted, because it is the first question in most care assessments
- Whether the medication went in, and the reason if it did not
What a good day depends on
Each day you can tick six things, and the Correlations tab compares the average on days each one happened against days it did not. In the worked example, agitation costs 2.8 points and eating well is worth 2.4. That turns a feeling into something you can raise at the next appointment.
Medications, appointments and results
- Medications: every dose change gets the average for the window before it and the window after, worked out
- Appointments: what was said, and what to ask next
- Results: lab results against the range your lab printed
- Report: the last thirty days against the thirty before, the factors ranked and the bloods against range, on one printed page for the doctor
Eleven connected tabs
Start Here, Setup, Daily Log, Wellbeing, Correlations, Medications, Appointments, Results, Monthly, Report and Dashboard. The worked example covers a year of elder care at home across 261 days, with a urine infection in April, a falls assessment and a care package in May, and the average good day rising from 4.5 to 7.1. A blank copy is ready for your own caregiver daily log, and it suits any family caregiver who needs a shared medication tracker and falls log.
This is a spreadsheet, not a medical device. It does not diagnose anything and it is not a substitute for advice from your doctor.

Frequently asked questions
Can I use it in Excel or Google Sheets?
Either. You download Excel workbooks for Excel 2016 or later on Windows and Mac, and the instruction guide has one-click links to make your own copy in Google Sheets with a free Google account.
Can several family members fill it in?
Yes. It is one row a day, written by whoever was there, so everyone can see what happened on the days they were not.
Does it record falls?
Yes. Falls have their own column and are counted, so you can answer the question straight away at an assessment.
Can I track whether medication was taken?
Yes. Each day records whether the medication went in and why not, and the Medications tab gives a before-and-after average on every dose change.
Is there a summary for the doctor?
Yes. The Report tab prints the last thirty days against the thirty before, with the factors ranked and the bloods against range, on one page.
