About this template

This lab results tracker keeps every blood test you have had in one place, so you can see how a value has moved over the years. Type each result with the range the lab printed beside it, and the file marks where it sits and shows the change since last time. It interprets nothing: ask your doctor what a result means.

Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).

The dashboard

Pick a test and its card, chart and history follow. Nothing on the page is typed.

  • How many latest results sit inside the range you typed
  • Every result of the test picked over time, against your range
  • Each latest result on a strip beside its range
  • Results inside and outside your range, draw by draw
  • When each test comes round next, by how often you have had it, and days until the next draw you booked
  • Which tests have sat outside your range most often
  • The history of the test picked, newest first, with the change each time

Every result beside its range

Each row of the blood test log holds the date of the draw, the test, the result, the unit and the range exactly as the lab printed it. The result is marked In range, Below range or Above range against that range, with the last result for the same test and the change.

The trend for every test

The Trends tab lists every test with its first and latest result, how often it sat outside its range and the five results before the latest. Pick one test and the dashboard charts its last eight results against the range, which makes it a handy cholesterol log or bloodwork tracker for any value you follow.

Every blood draw

The Test Dates tab lists each draw, newest first, with the lab, the reason, how many tests were run and the gap since the one before. Together it keeps your health records tidy enough to print for a medical binder or bring to an annual physical.

Light and dark editions

Every download includes a light edition and a dark edition. Every tab is restyled, not only the dashboard, and both run on the same formulas, so you simply pick the look you prefer. Each edition comes filled with a worked example and as a blank copy, in Excel and in Google Sheets.

Works with

  • Microsoft Excel 2016, 2019, 2021 and Microsoft 365, on Windows and Mac, and LibreOffice
  • Google Sheets with a free Google account. The Google Sheets version is a real workbook you take a copy of, not a PDF with a link in it.
  • No dynamic arrays, no macros, no add-ins and no subscription

What you download

  • Light edition: the Excel workbook filled with a worked example (83 results from nine blood draws over five years), and a blank one
  • Dark edition: the same two files in the dark design
  • The instruction guide, which has one-click links to the Google Sheets versions (light and dark, filled and blank)
  • The terms of use as a PDF

Your files are ready to download as soon as payment clears. They are for personal and business use on your own devices, so please do not share or resell them. Questions before or after you buy? Use our contact page and we will help.

Frequently asked questions

Does it work in Google Sheets?

Yes. The download includes Excel workbooks for Excel 2016 or later on Windows or Mac, and the instruction guide has one-click links to copy the Google Sheets versions with a free Google account.

Does it tell me what my results mean?

No. It interprets nothing. It compares each result with the range you typed from your lab report, and your doctor is the person to ask what a result means.

Which range does it use?

The one you type, as the lab printed it beside the result. Each result is marked In range, Below range or Above range against its own range.

Can I follow one test over time?

Yes. Pick a test and the dashboard charts its last eight results against the range, with its full history newest first.

What is the difference between the light and dark editions?

Only the look. Both have every tab restyled with the same formulas, and both are included.
