About this template
This food allergy symptom log works as an elimination diet tracker for anyone with several suspect foods and no clear way to tell them apart. You log what you ate and how you reacted each day, and the workbook does the comparison that most food diaries leave to guesswork.
An elimination diet only gives an answer if someone does the sums. Here they are done for you, every day you add a row.
Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).
Which suspect it is
The Correlations tab takes each of six suspect foods and compares your average reaction on the days you ate it against the days you did not, ranked by size. A real answer often looks lopsided: one food with a large effect and the others close to nothing. Removing six foods at once proves little on its own, but this view shows which ones you can put back.
Reintroduction, read the same way
When you bring a food back, the same columns show whether it was tolerated, so the reintroduction log works out the result instead of relying on memory. Skin, gut and breathing are scored separately, because a reaction can mean three different things and they do not always move together.
Everything for the appointment
- Room for 300 days, nine columns a day, with six renameable suspect foods
- Reaction days counted and colored
- Medications, with a before-and-after average on every dose change
- Appointments, with what was said and what to ask next
- Lab results against the range your lab printed
- A one-page report of the last thirty days to take to your doctor or dietitian
Eleven connected tabs
Start Here, Setup, Daily Log, Reactions, Correlations, Medications, Appointments, Results, Monthly, Report and Dashboard. The worked example follows a year of an elimination diet: six suspects logged daily, dairy out from June and reintroduced from August. Dairy turns out to be worth 5.3 points of reaction while the other five are noise. A blank copy is ready for your own food intolerance food diary and food trigger tracker.
This is a spreadsheet, not a medical device. It does not diagnose anything and it is not a substitute for advice from your doctor.

Frequently asked questions
Can I use it in Google Sheets as well as Excel?
Yes. You download Excel workbooks for Excel 2016 or later on Windows and Mac, and the instruction guide has one-click links to make your own copy in Google Sheets with a free Google account.
Can I change the suspect foods?
Yes. The six suspect foods can be renamed to whatever you are testing.
Does it help with reintroducing foods?
Yes. When you put a food back, the same with-and-without columns show whether your reactions changed, so the result is worked out rather than remembered.
Can I record different kinds of reaction?
Yes. Skin, gut and breathing are scored separately each day, and reaction days are counted.
Will it tell me what I am allergic to?
No. It records and compares what you enter and gives you a one-page report to discuss with your doctor or dietitian. It does not diagnose anything.
