About this template

This migraine tracker turns a daily headache log into the answers a doctor asks for. It is for anyone living with migraines or regular headaches who wants to walk into a short neurology appointment with attack counts, likely triggers and medication history already on one printed page.

Please read: This is a tracker tool only, not financial, tax, legal, medical or any other professional advice or advisory service. It records and calculates what you enter; you are solely responsible for checking all figures, rates, dates and results and for every decision you make. By purchasing, downloading or using it, you accept these terms (your statutory consumer rights are not affected).

What sets them off: the Correlations tab

Most migraine diary templates are a grid of days. This one does the subtraction. Name your triggers on Setup, such as sleep, alcohol, a missed meal, screens, weather or your cycle, and the file counts the days with each one, averages the pain on those days against the days without, and shows the difference.

  • Eight triggers compared, with the average pain on days with and without
  • A worth taking to your doctor marker on the triggers that move the numbers

In the worked example, the cycle adds 2.8 to the average pain and the weather adds nothing.

A daily log that takes seconds

The Daily Log holds 400 days of entries for pain, hours, light sensitivity, nausea, sleep and stress, so it works as a broader symptom tracker too.

  • Attack days counted against a pain threshold you set
  • The last 30 days shown beside the 30 days before, so whether it is getting better has a number

Medications, appointments and results

  • Medications: five medications with dose, days on and refill dates
  • Appointments: what was said and what changed
  • Results: test results against their reference range
  • Attacks and Monthly: attack days and month-by-month totals
  • Dashboard: reads all of it
  • Start Here and Setup: setup in a few minutes

One page for the appointment

The Report tab fits on a single sheet of paper, ready to print as a neurologist report. It shows the time window, the averages, the attack count, the window before it and the trigger table. Nothing on it is typed: every figure comes from your headache journal entries.

This is a spreadsheet, not medical advice or a medical device. Take it to your doctor; do not use it instead of one.

Works with

  • Microsoft Excel 2016, 2019, 2021 and Microsoft 365, on Windows and Mac, and LibreOffice
  • Google Sheets with a free Google account. The Google Sheets version is a real workbook you copy, not a PDF with a link in it.
  • No dynamic arrays, no macros, no add-ins and no subscription

What you download

  • Two Excel files: a worked nine-month diary with 261 days and 71 attack days, where a preventative started in April brings the attack rate down from fourteen a month to four, and a blank copy
  • The instruction guide, which has one-click links to the Google Sheets versions
  • The terms of use as a PDF

Your files are ready the moment payment clears. The log is for personal and business use on your own devices, so please do not share or resell the files. Questions? Reach us through our contact page.

Frequently asked questions

Do I need Excel, or can I use Google Sheets?

Either. The download has Excel files for Excel 2016 or later on Windows and Mac, and the instruction guide has one-click links to copy the Google Sheets versions with a free Google account.

How does it work out my triggers?

You name your triggers on Setup and mark them each day. The Correlations tab averages the pain on days with each trigger against days without and shows the difference.

Can I print something for my doctor?

Yes. The Report tab is a single printable page with the averages, attack count, the previous window for comparison and the trigger table, all calculated from your log.

What counts as an attack day?

You set a pain threshold, and any day at or above it is counted as an attack day.

Is this medical advice?

No. It is a spreadsheet, not medical advice or a medical device. Take it to your doctor rather than using it instead of one.
